Core Item Observed the walk in cooler door no longer closing properly, it is unable to seal the walk in cooler. The temperature of the walk in cooler is still in the safety zone for temperatures.

Priority Foundation Item Evidence of pests. Observed a cockraoch in the area next to the walk in cooler on a panel that was down, it was also observed that the facility has an excessive amount of drain flies in the bar area..
Provide proper pest control by trapping or otherwise controlling pest problems on a continual basis. Clean up all pest activity evidence and increase control activities. Correct By: 15-Oct-2020
6-501.12 / CLEANING, FREQUENCY AND RESTRICTIONS
Core Item The walk in cooler has mold growing heavily on the walk in freezer door and on the back wall. There is excess debris throughout the facility, in the dry storage area, on the can racks and other areas.
Clean the physical facilities at a frequency necessary to keep them clean. Correct By: 16-Oct-2020

2-401.11/EATING, DRINKING, OR USING TOBACCO
Drinking cup without cover was observed in food preparation area.
Provide approved beverage container in food preparation area. Correct By: 08-Oct-2020 Action Taken: Staff now has policy on cups having lids and straws. No open drinks were observed in the kitchen area.
3-305.14/UNPACKAGED FOOD PROTECTED FROM CONTAMINATION DURING PREPARATION
The corn meal in the dry storage area is torn open and open to possible contamination.
Remove food from this location or provide alternate methods to protect food from contamination. Correct By: 09-Oct-2020 Action Taken: All food was properly covered and stored in the dry storage area.
4-602.11 (E)/EQUIPMENT FOOD-CONTACT SURFACE
Observed slime mold growing on the panel of the ice machine.
Clean the ice machine at a frequency that prevents the development of debris. Correct By: 09-Oct-2020 Action Taken: The ice machine was deep clean and was observed without slime mold.
5-205.11/USING A HANDWASHING SINK
Handsink being used for storing utensils during inspection and is unavailable for proper handwashing.
Train food employees that handwashing sinks shall not be used for any other purpose than handwashing. Correct By: 09-Oct-2020 Action Taken: All hand sinks were labeled as hand sinks only and were free of excess utensils.
6-501.110/USING DRESSING ROOMS AND LOCKERS
Observed staff clothes and purse in the dry storage area.
Provide dressing rooms for employees to change their clothes. Correct By: 16-Oct-2020 Action Taken: All staff clothes and accessories are now stored upstairs away from kitchen area.
6-501.14/CLEANING VENTILATION SYSTEMS, NUISANCE, AND DISCHARGE PROHIBITION
The air ducts and filters are covered in a layer of debris.
Clean intake exhaust air ducts, exhaust air ducts and filters so they cannot be a source of contamination. Correct By: 15-Oct-2020 Action Taken: The air ducts and filters were deep cleaned and observed without any grease build up.
Comments
This was a follow up inspection following the office conference, the facility corrected most of the violations that were observed in the previous inspection. For the pest control violation, the facility has provided invoices and proof of work from their pest control operator (Terminix). Progress has been made with the pests and work will continue to eliminate the remaining pests. The good repair violation has also been addressed, the facility has made the order for a new walk in cooler door and gaskets for reach in coolers, this violation will be corrected upon the installation of the new door and gaskets. The cleaning frequency violation is also being addressed, the facility has made strides in cleaning the facility and more work is being done with deep cleaning throughout the facility.
