TWO PRIORITY VIOLATIONS WERE CITED AND CORRECTED. ONE PRIORITY FOUNDATION VIOLATION WAS CITED. TWO OF THE VIOLATIONS WERE IN RELATION TO DATE MARKING AND LACK OF DISCARDING EXPIRED FOODS. ONE VIOLATION WAS IN RELATION TO A PIECE OF EQUIPMENT THAT WAS NOT FUNCTIONING PROPERLY.

A FOLLOW-UP INSPECTION SHALL BE CONDUCTED IN TWO WEEKS TO ASSURE THAT THE DATE MARKING SYSTEM HAS BEEN IMPROVED, FOODS PAST THEIR DATE OF DISPOSITION ARE DISCARDED, AND THE MALFUNCTIONING PIECE OF EQUIPMENT HAS EITHER BEEN REPAIRED OR TAKEN OUT OF SERVICE.

FURTHER, THE CERTIFIED FOOD MANAGER'S CERTIFICATE HAS EXPIRED. EITHER HAVE A COPY OF A VALID CERTIFICATE AVAILABLE FOR THE INSPECTOR AT THE FOLLOW-UP INSPECTION, SIGN UP FOR A CLASS/EXAM TO BECOME RECERTIFIED, OR HIRE A PERSON TO BE THE CERTIFIED FOOD MANAGER ON STAFF WITHIN THREE MONTHS.
