4-501.11/GOOD REPAIR, PROPER ADJUSTMENT, AND OPERATION
Repeat X2: Bison cold-top reach-in does not maintain proper holding temperatures as ambient temp is 47-53F towards front due to right door does not properly close tight, has broken gaskets and is taped to keep gasket on. There is ice build-up on back due to moisture entering from the loose door without a tight seal. Repeat 3X: Unit has been partially fixed but the doors do not close tightly and loosen off when opening.
Maintain equipment in good repair. Correct By: 21-Oct-2019 Action Taken: Unit is fixed and ambient temp is 38F. New gaskets and screws into doors to shut close.
3-305.11/FOOD STORAGE
REPEAT X3: Onion bag, onion crate, several cases of potatoes stacked and stored on the floor in the backroom and walk-in cooler (2) boxes of head lettuce and box pita bread, .

Plastic cup lids in a open box are being improperly stored on the floor.
Store single-use and single-service articles in a clean, dry location a minimum of 6-inches off the floor. Correct By: 11-Oct-2019 Action Taken: Food containers are being properly stored onto shelves.
