4-903.11 (A), (C) / EQUIPMENT, UTENSILS, LINENS, AND SINGLE-SERVICE AND SINGLE USE ARTICLES
Core Item 1)Clean items are on cloth towels, both at the bar & in the kitchen. 2)Clean items are in a dirty drawer, in the stainless steel table to left of the kitchen handsink.
Store single-use and single-service articles in a clean, dry location a minimum of 6-inches off the floor. 1)Cloth absorbs moisture, attracts bacteria, grows that bacteria, & grows it. Remove items from the cloth towels & put them simply on a tray or a paper towel. 2)Wash, rinse, sanitize,& air dry the drawer insert as well as all utensils in it, before they are returned to it. Correct By: 24-Feb-2015.
5-205.15 (B) / SYSTEM MAINTAINED IN GOOD REPAIR (REPAIRED)
Core Item *REPEAT--REPEAT* The drain lines for rthe bar's 3-bay sink - sanitzing bay, which is the skinny PVC line - is cut off, but it is laying down in the drain. This is an indirect connection by means of an air break.
An air gap, the highest form of protection, is required. Cut off the line so that it is compelety up out of the floor drain by at least 1". This is to protect the wares/dishes in the event of a backup. Correct By: 17-May-2015.
6-304.11 / MECHANICAL
Core Item *3RD REPEAT* There is no mechanical ventilation in the men's restroom.
Provide adequate mechanical ventilation of sufficient capacity that would remove excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes. NOTE: owner states that windows with screens will be installed when the weather breaks. Windows are on site. Correct By: 17-May-2015.
6-501.111 (A-B), (D) / CONTROLLING PESTS
Core Item There is a dead mouse in a glue board just outside the north storage walk-in cooler (WIC). There are old mouse droppings on the saw table & the stainless steel "table" next to it, in the storage room there.
Remove all unnecessary items from this area to minimize harborage for all pests. Get all kept items up off of the floor at least 6", on racks/shelves that are smooth & easily cleanable, & that can be seen & cleaned under. Remove dead mouse glue board. Clean up old droppings & monitor area. Correct By: 24-Feb-2015.

--CATHY WAS THE PIC AT A ROUTINE OPERATIONAL EVALUATION CONDUCTED TODAY. DENNIS, OWNER, WAS ALSO PRESENT. FOUR (4) CORE VIOLATIONS WERE CITED, TWO (2) OF WHICH ARE REPEATS.
--THIS FACILITY WAS AGAIN FOUND TO BE CLEAN & NEAT, WITH A NEW CEILING INSTALLED IN THE "BACK" KITCHEN/STORAGE AREA.
--THERE IS A HEALTH POLICY IN EFFECT. THE KITCHEN ARCTIC AIR COOLER IS NOT WORKING & HAS NOT BEEN IN USE.
--THE PIPELINES FOR THE KITCHEN 3-BAY SINK WAS ABLE TO BE SEEN - IT IS AIRGAPPED AS IS REQUIRED.
