Adjust method or procedures so that all ready-to-eat potentially hazardous foods are date marked with a date not to exceed 7-days. Correct By: 18-Oct-2018 Action Taken: Items in reach-in cooler cold-top holds items used within 24 hours ham slices, cook sausage, and deli turkey meat and other foods requiring date-marking approved.
4-101.11 (B-E)/CHARACTERISTICS - FOOD CONTACT SURFACES EQUIPMENT AND UTENSILS
1) Plastic tubs from a commercial packaged food were being reused for hot-holding lemon rice soup. 2) Egg crates being used in hot-hold pans for ready-to-eat foods: cooked bacon and hot dogs.
DISCONTINUE THIS PRACTICE - THIS PLASTIC IS NOT DESIGNED FOR THIS USE AS THE CONTAINER MATERIALS MAY ALLOW HARMFUL CHEMICALS TO BE TRANSFERRED TO FOOD THAT COULD POTENTIALLY LEAD TO FOODBORNE ILLNESS. **EGGS CAN CARRY SALMONELLA AND CAN TRANSFER THE PATHOGEN AND OTHER MICROBES THAT CAN CAUSE FOODBORNE ILLNESS FROM THE TRAYS TO THE COOKED FOODS.** Correct By: 18-Oct-2018 Action Taken: Egg crates and plastic tubs removed as food-contact equipment.
6-501.11/PHYSICAL FACILITIES - GOOD REPAIR
The physical facilities in dining ceiling tiles are missing and large torn holes present that open to exposing attic to the area.
Maintain the physical facilities so they are in good repair at all times. Correct By: 29-Oct-2018 Action Taken: Work on roof in progress and holes in ceiling repaired. * Pictures of ceiling and roof work will be placed in file.

Deli slicer is visibly soiled and was last used at unknown time.
Maintain food contact surfaces in a clean condition. Correct By: 18-Oct-2018 Action Taken: Slicer was used at 7am and will be cleaned 11am or sooner per employee.
