Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Repeat The plumbing system was not properly repaired. Hand washing sink facuet is leaking.
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code.

Equipment and/or components are not maintained in good working order. Both of the front hot holding cabinets are only holding at 120 F.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
Comments
No food in the hot box by the wall, hot box by the prep line was holding at 135 F. Please obtain thermometers for these units as the built-in thermometers do not appear to be accurate.
