3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection The handwashing sink was not easily accessible. Toys in boxes stored in front of hand sink near drive through.
To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use.
3717-1-07.1(A) / Poisonous or toxic materials: Storage: separation.
Critical Corrected During Inspection Observed improper storage of poisonous or toxic materials. Chemical cleaners (grill cleaner, degreaser) stored on shelf above top-open burger freezer.
To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles by: 1) Separating the poisonous materials by spacing or partitioning; and 2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles or single-use articles. .

3717-1-04.1(Y) / Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Observed two coolers (breakfast items on grill line and beverage cooler near drive through) without any thermometers.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. Nugget freezer has very heavy accumulation of frost.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Boxes of singe-use cups stored on floor in drive through hallway and near walk in cooler.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.

Received a complaint stating: employees do not wear any gloves as required for food prep. Trainer told complainant that she knows she is supposed to wear gloves, but does not. She sanitizes her hands instead. Same trainer when making parfaits was also eating while preparing customer's food - in between running out and smoking cigarettes. Entire kitchen area in which employees are located extremely dirty. Everything is greasy. Headsets wore in drive-thru have disintegrated, disgusting foam pieces. Shared among employees.who do have their own foam insert. Bathrooms disgusting. Dining area where customers get drinks is not cleaned. Fruit flies everywhere.

Spoke with manager of facility. Discussed proper ways to prevent bare hand contact and proper personal hygiene (eating while preparing food). Manager is aware of proper procedures and will speak to staff. Facility was clean, and only a couple of fruit flies were observed near the drive through area where milkshakes are made. Discussed further cleaning of this area to prevent any sort of pest attraction. Bathrooms were clean, as was the dining area. Complaint was not valid at time of inspection.

A standard inspection was conducted at the time.
Discussed cleanup procedure for diarrheal/vomitus incidents. Manager will ensure it is available for review.
