A summary of the violations found during the inspection are listed below.

43.1-3-4(a)-(h)   The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
6-301.14  Corrected During Inspection Observed that sign or poster that notifies food employees to wash their hands is not provided at bathroom handwashing sink used by food employees.
Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.
4-501.114(C)  Corrected During Inspection Critical When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3 vat sink with a concentration of 0 ppm total quaternary ammonium compound. CFM corrected quat solution to 200 ppm.
The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
7-202.12(A)(1)-(4)  Corrected During Inspection Critical Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests: can of roach spray. cfm discarded item
Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination.
4-601.11(C)   The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: true 2dr cooler
Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
6-501.110(A)   Employees are not using the dressing rooms or lockers provided.
Instruct employees to use the designated dressing rooms to prevent contamination of food, food equipment and utensils, and preparation surfaces.
