2-201.11(A)(1)-(5)   Critical Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Employee reporting agreements had not been signed.
A complete employee health policy must have the following elements: 1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout); and 2) Documentation that employees have been instructed of their responsibility to report symptoms of, diagnosis of or exposure to foodborne illness to management (see "Employee Illness Reporting Agreement" handout); and 3) A management plan to restrict or exclude employees, as applicable, who have symptoms, diagnosis or exposure to foodborne illness (see "Employee Illness Decision Guide for PIC/CFM" handout). The information provided at the time of this inspection will help you develop and implement this policy. Handouts are available in the following languages: English, Arabic, Chinese (traditional), Farsi, Hindi, Korean, Spanish, Thai, Urdu and Vietnamese. 3-301.11(C)   Excessive bare hand contact with exposed food that is not in a ready-to-eat form. Plastic containers used in flour/corn meal bulk containers rather than scoops with handles. NOTE: Discussed with PIC who will provide scoops with handles.
Food employees shall minimize bare hand and arm contact with exposed food that is NOT in a ready-to-eat food. Foods like sugar, salt, flour, starch, etc. shall be dispensed using food scoops or utensils that are designed with handles to minimize bare hand contact.
3-302.11(A)(4)  Corrected During Inspection Critical Unwrapped or uncovered food in the following location where the food is subject to contamination: sacks of flour and cornmeal. NOTE: PIC provided cover for corn meal and flour.
Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled.
4-601.11(A)  Corrected During Inspection Critical The following equipment/utensils were observed soiled to sight and touch: can opener. NOTE: PIC had can opener cleaned, rinsed and sanitized.
Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
3-501.19(B)(1)-(4)  Corrected During Inspection Critical The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Cheese Pizza. NOTE: PIC created written record with time pizza must be disposed.
If time only, rather than time in accordance with temperature control, up to a maximum of 4 hours, is used as the public health control the food shall: 1) have an initial temperature of 41°F or less if removed from cold holding temperature OR 135°F or above if removed from hot holding temperature control, 2) be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control, 3) be cooked and served, served if ready-to-eat, or discarded within 4 hours from the point in time when the food is removed from temperature control, and 4) be discarded if unmarked or marked to exceed a 4 hour limit.


The purpose of this visit was to conduct a routine inspection. Water Heater AO Smith BT80. : Pest control: 1x/month, invoice dated 1/23/13 on file. Hood service: grease extractors cleaned 1x/week; hood serviced every 3-4 months invoice dated 12/26/12 on file. One dr True undercounter cooler in front serving area was out of service, unplugged, and empty at the time of the inspection. PIC advised that a service inspection had already been scheduled, and that the cooler would remain out of service until restored to fully functional condition. PIC advised that she will have all employees sign the EHP employee illness reporting agreement. PIC will provide fully functional thermometers in the front of all cooling units, and scoops with handles in all bulk contianers.
