6-501.11 / PHYSICAL FACILITIES - GOOD REPAIR Ceiling in womens toilet room is water damaged. Maintain the physical facilities so they are in good repair at all times. Manager stated that they are working on the problem to repair the roof and ceiling at this time.
6-501.11 / PHYSICAL FACILITIES - GOOD REPAIR Wooden floor board outside walk-in cooler is no sealed/covered and has mold accumulation. Maintain the physical facilities so they are in good repair at all times. Seal board so that it is nonabsorbent, smooth and easily cleanable.

Note: Please provide shelving in the walk-in cooler that allows air to flow freely around the food. Current shelving is for dry storage and not for refrigeration storage. Note: Waffle batters must be kept at 41F. If using 4-hour time instead of temperature to control the growth of bacteria you must maintain a written method to keep track of the time.
