2-102.11 (A)(B)(C 1, 4-16)/DEMONSTRATION OF KNOWLEDGE PIC was unable to provide proof of Food Safety Manager certification. PIC provided the Food Safety Handler certificate INSTEAD of the Food Safety Manager certificate. PIC shall register for the Food Safety Manager (FSM) course within 14 days. PIC shall pass the FSM exam within 30 days. PIC shall provide proof of FSM certification via email within 30 days. Correct By: 14-Jun-2023 Action Taken: PIC provided FSM on 07/14/23 via email. Action Taken: Copy of FSM certificate is attached to report.
