Wash, rinse, and sanitize noted item every 4 hours.
Action Taken: PIC washed, rinsed, and sanitized noted item.

Core Item Observed several ceiling tiles located above dishwasher and food area with excessive dust. Observed wall behind 3 compartment sink that is visible with food debris, dust build up, and mold.

Priority Foundation Item Corrected During Inspection Soap dispenser in the back room was not dispensing soap. Therefore, soap was not available at food employee handwash sink.
Provide hand soap at handwashing sink to facilitate proper handwashing.
Action Taken: PIC provided noted item.
