2-102.11 (A)(B)(C 1, 4-16) / DEMONSTRATION OF KNOWLEDGE Priority Foundation Item PIC was unable to provide proof of Food Safety Manager certification. PIC provided the Food Safety Handler certificate INSTEAD of the Food Safety Manager certificate. PIC shall register for the Food Safety Manager (FSM) course within 14 days. PIC shall pass the FSM exam within 30 days. PIC shall provide proof of FSM certification via email within 30 days. Correct By: 14-Jun-2023

Comments
Spoke to PIC via email regarding the proof of the Food Safety Manager certification. PIC enrolled in the FSM course on 05/18/23 but has not yet taken the exam. Proof of enrollment is provided. Local sanitarian spoke to EH supervisor to allow a 30 day extension to pass the exam and provide the FSM certificate via email. PIC shall provide proof of certification by 07/14/23.
