3701-31-04 (B)(6)(a) / Pool structure is water tight
The pool bottom was in disrepair (large areas with chipped paint). Repair the pool structure to a water-tight system. If necessary, consult with the Ohio Department of Health regarding the repair. Discussed with the person in charge.

3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Observed both sets of ladders were not secured well to the pool area. Secure the ladders so that they are slip resistant and in good repair. Discussed with the person in charge.

3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Observed planks of the deck surface to be unsecured to the floor. In order to reduce the risk of tripping, ensure that these planks are adequately attached. Discussed with the person in charge.

3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
Corrected During Inspection Observed a USCG Type IV personal flotation device with a 30-60 foot throw line that was not attached to the flotation device. Provide throw lines that are attached to flotation devices near the pool at all times. Discussed with the person in charge, who attached the line appropriately.

3701-31-04 (E)(2)(b)(i) / 12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
Observed a 12 ft. telescopic reach pole with Shepherd's crook was provided at the guarded pool. Provide the non-telescopic reach pole device at the pool at all times. Discussed with the person in charge.

3701-31-04 (E)(2)(d) / First aid kit provided
Observed a first aid kit with no items to slow or stop bleeding. In order to reduce the risk of the spread of pathogens, ensure that the first aid kit has both gloves and items to stop bleeding. Discussed with the person in charge.

3701-31-05.1 M(3)- Observed chipped tiles around the areas near both 4ft markers of the pool. In order to prevent physical harm, ensure that areas that edges/coping are smooth.

Observed several dates in the records that were left blank. Discussed with person in charge, who stated that the pool was closed on those days for maintenance. Discussed the importance of adding a note about maintenance in the records in the future.
