3701-31-04 (D)(7)(c) / Audible/visual alarm shall activate when unit is malfunctioning or when pH or ORP levels are not acceptable.
Critical Observed that the ACC was in alarm.
In order to reduce the risk of possible health or safety hazards, ensure that the ACC is no longer in alarm and properly functioning. Have the ACC unit serviced or replaced so that the audible or visual alarm is activated when necessary.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
Observed a personal flotation device with a throw line that is less than one quarter inch line.
In order to reduce the risk of a safety hazard, ensure that a USCG Type IV personal flotation device with 30 -60 foot throw line (one quarter inch diameter) is provided at the unguarded pool. Provide a proper floatation device at all times.

Comments
Tested the pH of the pool and it was 7.3. The ACC was still in alarm. The ACC is set to 7.4 but says that the pH was reading at 4.54. Please have EcoLab come back out and service ACC. Send photo of ACC to Eugene with Ecolab.
