Repeat Observed missing tiles around the top edge of the pool.
Repair the pool structure to a water-tight system. If necessary, consult with the Ohio Department of Health regarding the repair.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Doors were not self-closing and lockable. Observed the door to the outside to not be self locking when it closed.
Repair outside door in order to be self-closing and locking.
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Observed the emergency device (telephone) to not be working. PIC diconnected the phone and reconnected it and the phone began working again. PIC stated they had maintenance done on the phones earlier in the day.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
