Observed the door leading to outside of the building was not fully closed. In order to reduce the risk of patron harm, ensure that perimeter barriers are utilized in a way to prevent unauthorized entry.

3701-31-04 (D)(7)(b) / pH and ORP values shall be displayed.
Critical Repeat Observed ORP value was not displayed on the ACC unit. Discussed with person in charge. Ecolab is waiting on the part to become available to correct the issue. Have the ACC unit serviced or replaced so that the ORP value is displayed when the parts are available.
