3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Repeat The facilities were not maintained in a safe, clean & sanitary, and in good repair. The pool chemical storage closet and the pool pump room were locked and work had been done to correct violations. They were less disordered, with fewer junk items scattered on the floor, no empty chemical bottles strewn on the floor, better-organized storage shelves, and cleaner floors. One bag of sodium bicarbonate was still open and stored on the floor of the chemical storage room. In order to reduce the risk of accidents, falls, and misuse or accidental spillage of chemicals, it is necessary that storage closets be kept clean and well-organized. 3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Repeat Equipment was not maintained per manufacturers specifications. One of the hooks for the shepherd's crook had been srewed back onto the wall but was pulling back out during the inspection. Repair the wall, replace the hook with a proper connector or in a stronger portion of the wall, and rehang the shepherd's crook.

3701-31-04 (B)(6)(j) / All SVRS devices are maintained/ functional and tested
Critical Repeat There was no record that the Stingl Switch had been checked in the last 30 days. In order to ensure the proper function of the switch and to reduce the risk of serious injury, it is necessary that the switch be checked for proper function at least once every 30 days. Discussed with the PIC who indicated that he had checked the switch but forgotten to make a note on the log. Always make a note of the times when the switch is checked.

3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. One of the ladders is still marked off with caution tape. In order to provide adequate ladders to allow entrance and exit from the pool, it is necessary that all ladders are in good condition and available for use. Repair or replace this ladder.

Repeat Deck surfaces had standing water or other hazards. Several floor tiles on the pool deck are still broken and angled up, causing a trip hazard. The area is currently covered with pool furniture - mark it well with caution sign. In order to reduce the risk of accidents, it is necessary that the pool deck be kept free of hazards. Repair this area.

3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical Repeat The pool pump room is locked but still has a broken section of PVC pipe (described as the manifold) in the circulation system. Replace all cracked or damaged PVC pipe to stop the water leak and dry the floor. Discussed with the PIC, who showed the inspector the replacement manifold, which was in a box and scheduled to be replaced as soon as possible.

3701-31-04 (B)(6)(u) / Ventilation system is maintained/ free of hazards
Repeat The metal screen for ventilation between the workout room and the pump room is still broken. In order to maintain proper ventilation in the pump room, the metal screen must be open and in good condition. Replace the broken grill and secure it in place with appropriate fasteners.

The licensee /authorized representative was not keeping or maintaining operational records. No daily logs were being kept for the facility. In order to ensure proper pool maintenance, it is necessary that chemical levels be measured and recorded and that all corrective actions be logged. Discussed with the PIC. Always keep daily logs.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(6)/Equipment is maintained per manufacturers specifications.
Equipment was not maintained per manufacturers specifications. The Pool Pilot Digital "check system" light is flashing and the read out indicates that salt should be added. In order for the unit to function properly, it must be supplied with adequate amounts of salt. Check and restock this unit, if needed.
All equipment must be maintained in accordance with the manufacturers specifications.
3701-31-04 (B)(6)(h)/Required meters and gauges maintained/functional
Required meters and gauges were not maintained or were not functional while the pool is in operation. The sulfuric acid dispensing tubing is not correctly attached to the pH feeder and adequately controlled by the automatic chemical controller. In order to ensure adequate control of pH, it is necessary that the sulfuric acid supply is correctly attached to the feeder and controlled by the controller. Repair the sulfuric acid dispensing system.
Maintain all meters and gauges in a properly functioning manner at all times while the pool is in operation.
3701-31-04 (D)(8)/Acceptable test kit provided/adequate/in good repair
The comparator tube for the pool test kit is discolored and difficult to use for color comparisons. In order to correctly check the color changes that are part of pool testing, it is necessary to have a comparator tube that is clean and transparent. Replace this comparator tube.
3701-31-04 (E)(2)(a)(ii)/A USCG Type IV personal flotation device with 30 -60 foot throw line provided
The personal flotation devices were available but did not have their throw lines attached. In order to correctly use the flotation devices, their throw lines have to be attached. Attached the throw lines to these flotation devices.
3701-31-04 (E)(2)(e)/Working emergency telephone/call box provided.
Emergency device (telephone) was not working. The emergency telephone does not work - if one presses "0", it does not connect to the front desk. When one presses "9-1-1", it does not connect to emergency services. In order to ensure access to help in an emergency situation, it is necessary that the telephone function. Repair or replace this telephone.

The Stingl Switch appeared to be functioning, but had not yet been checked. In order to ensure adequate function of the vacuum release switch, it is necessary that the Stingl Switch is fully operational. Discussed with the PIC. Check the manual and confirm that the switch is working properly.
Comments
THE OPERATOR VOLUNTARILY CLOSED THE POOL UNTIL HE COULD MAKE THE REPAIRS NEEDED TO TURN THE PUMP BACK ON.

Discussed the maintenance scheduled for the pool with the manager, Maurice Herald, and the pool operator, Dusty Connell. They indicated that they intended to order and purchase several replacement parts and start repairs on several areas as soon as possible and would keep the pool closed until they are finished.
No repairs require approval from the Ohio Department of Health.

The Health District will check on progress at the follow-up inspection on October 16, 2020.
Please call the Health District with any updates.

Ensure that the following issues are resolved at the follow-up inspection:
1. Keep cleaning the pool chemical storage room until all of the pool chemicals are on shelves and well-organized.
2. Rehang the support hook for the shepherd's crook so the crook can be securely attached to the wall.
3. Repair the pool ladder.
4. Repair the tiles on the pool deck.
5. Replace the leaking portion (manifold) of the PVC in the pump room so the system can be turned "on".
6. Reattach the Stingl switch to the pool circulation system. .
7. Replace the damaged screen in the pump room.
