3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat SVRS/APSOS/ACC test results/ service records were not maintained. The ACC had been checked and calibrated - thank you. The SVRS had not yet been checked. In order to ensure safe use of the pool and proper monitoring of the pool chemistry, it is necessary that the SVRS and ACC be checked at least once per month. The PIC indicated that he would learn how to check the device and check it in the next month.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.

3701-31-04 (B)(4)/The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining complete operational records. Observed that the daily logs did not have entries for all days when the pool is open or for all actions needed. In order to ensure safe water for swimming, it is necessary that the pool be checked properly and the pool log be completed every day that the pool is open.
All facility records must be maintained and available for review upon request. Always complete the daily logs every day that the pool is open.
3701-31-04 (B)(6)(e)/Equipment identification labels are maintained or on file
Equipment identification labels were not maintained and/or not on file. The sand filter did not have the manufacturer's label on its side. A type-written list of specifications had been provided. In order to reduce the risk of accidents, it is necessary for equipment to have original labels.
Replace the missing equipment labels or have the proper equipment information on site for each piece of equipment.
3701-31-04 (B)(6)(h)/Required meters and gauges maintained/functional
Required meters and gauges were not maintained or were not functional while the pool is in operation. Observed the flow meter to be not working and to contain many bubbles. In order to ensure proper flow, it is necessary that the flow meter be kept in good working condition.
Maintain all meters and gauges in a properly functioning manner at all times while the pool is in operation. Repair or replace this flow meter.
3701-31-04 (B)(6)(o)/Ladders/stairs/steps are in good repair/slip resistent
Ladders/stairs/steps were not in good repair or were not slip resistant. Observed one of the ladders on the wide portion of the pool to be very loose from the pool wall. In order to reduce the risk of an accident, it is necessary that the pool ladders be well-secured to the pool.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (E)(2)(a)(ii)/A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Observed the flotation device to be hung inside a wall-hung wooden cabinet with a broken door (with a glass insert) and a sign telling patrons to not open the door. When the door was opened, the glass hung away from the door and posed the risk of broken glass. It is necessary that the flotation device be available for use and that equipment be in good repair.
Provide a proper flotation device at all times. Repair or remove the cabinet.
3701-31-04 (E)(2)(b)(i)/12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
12 ft. non-telescopic reach pole with Shepherd's crook and/or a USCG Type IV personal flotation device with 30 - 60 foot throw line was not provided at the guarded pool. Observed a telescopic reach pole at the pool. It is necessary that the Shepherd's crook reach pole be NON-telescopic.
Provide a non-telescopic reach pole and personal floatation device at the pool at all times.
3701-31-04 (E)(2)(d)/First aid kit provided
First aid equipment was not provided. Observed an empty first aid kit hung near the pool. In order to allow first aid, it is necessary that a stocked first aid kit be provided near the pool.
Provide first aid equipment, including a first aid kit, at the pool at all times.
