Required meters and gauges were not maintained or were not functional while the pool is in operation. The flow meter was not functional - the float was "stuck" in one place inside the tube.
In order to ensure a proper flow rate, it is necessary that the flowmeter be maintained in a properly functioning manner at all times while the pool is in operation. Clean the flowmeter or replace the flowmeter.
