[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(1)(e) / Automatic Chemical Controller (ACC) shall function properly.
Critical Observed automatic Chemical Controller (ACC) was not functioning properly at the time of the inspection
Do not operate the pool when the ACC unit is not functioning properly. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation. Discussed with person in charge, who said they would call the chemical dispenser company.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Observed the licensee /authorized representative was not keeping or maintaining operational records on Sundays.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Observed SVRS/APSOS/ACC test results/ service records were not maintained.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.
3701-31-04 (B)(6)(a) / Pool structure is water tight
The pool bottom was in disrepair (large areas with chipped paint).
Repair the pool structure to a water-tight system. If necessary, consult with the Ohio Department of Health regarding the repair.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Observed required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(k) / Outlet drain covers are secure/meet ANSI/ASME A112.19.8-2007 requirements.
Critical Observed two drains without covers.
In order to reduce the risk of physical harm to swimmers, ensure that appropriate covers are secured over the drains at the bottom of the pool. Discussed with person in charge, who positioned "Pool closed" signs until the facility is able to obtain drain covers.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Observed ladder was not secured well to the pool area.
Repair or replace the ladder so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Observed the perimeter gate near the spa were not effectively self-latching.
In order to reduce the risk of patron harm, ensure that perimeter gates are self-latching.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Observed pH was outside the range of 7.2 to 7.8 (recorded at 7.0).
Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (E)(2)(b)(i) / 12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
Observed a 12 ft. telescopic reach pole with Shepherd's crook was provided at the guarded pool.
Provide the non-telescopic reach pole device at the pool at all times.
3701-31-04 (E)(2)(d) / First aid kit provided
Observed first aid equipment was not provided.
Provide first aid equipment, including a first aid kit, at the pool at all times.
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Observed emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
