[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Observed that the testing records for the spa do not reflect that the spa is being tested with the required frequency.
All facility records must be maintained and available for review upon request. The bromine must be checked before the spa is used and at least every 12 hrs thereafter. The pH must be checked befoer the spa is used and at least every 4 hours thereafter. The water temperature shall be checked dialy. The alkalinity shall be checked once per week. Please revise testing requirements and record keeping to reflect this schedule.
3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Flow meter is not functioning.
Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation. Repair/replace the flow meter so that you can determine the flow rate of the spa.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat The permiter gate does not self close.
Repair/replace the gate so that it self closes and latches.
3701-31-04 (B)(6)(v) / Electrical components are maintained/ free of hazards
Critical Observed that the switch cover was missing for the blower motor controls.
Electrical components shall be maintained in a safe fashion. Replace the cover for the switch.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat Observed the pH to be 8.0.
The pH shall be maintained between 7.2 and 7.8 at all times. Adjust the pH to comply with rule.
3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was observed at 40 ppm.
Total alkalinity shall be over 60 ppm.
3701-31-04 (C)(7) / Spa water temperature is less than 104 degrees Fahrenheit/ thermometer is provided
Repeat Water temperature observed at 106 degF.
Spa water shall be maintained at 104 degF of less at all times. Reduce the water temperature.

Observed that the pH in the spa was 8.0 but the auto chemical controller was reading at 7.4 to 7.6. It may be that your pH sensor needs to be calibrated. Please look into correcting this as problems such as this would explain your chronic issues with proper pH.
