[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Observed that the time of testing the pool was not recorded on the logs. Observed that the pool was not being tested every 4 hours.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. Record the times at which the pool is being tested so that we can verify that it being tested before bathers enter and thereafter at the appropriate frequency
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Observed that combined chlorine and total chlorine was not recorded daily on the logs.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. Record the combine chlorine and total chlorine on the daily logs.
3701-31-04 (B)(4)(a)(iv) / Water temperatures are recorded daily
Observed that the water temperatures were not being recorded daily.
Water temperature shall be recorded not less than once per day. Take and record the water temperature at least once/day.
3701-31-04 (B)(4)(a)(v) / Water clarity are not present
Observed that water clarity was not being recorded on the daily logs.
Water clarity shall be assessed continuously and shall be recorded on the logs. Do not operate the pool while the water is unclear. Determine cause of the lack of clarity and immediately correct the issue prior to bathers using the public swimming pool.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Observed that there was no backflow prevention device on the hose bibb in the pump room.
Cross connection hazards shall be prevented. Remove all cross connection hazards from the pool area. Install the proper backflow prevention device on this hose bibb.
3701-31-04 (D)(02) / Public swimming pool is continuously disinfected using a chemical feeding device connected to the circulation system
Critical Observed that the pool did not have a chemical feeding device. The operator has been putting chlorine tablets into the strainer basket of the pump.
Pools shall be continuously disinfected using a chemical feeding device conected directly to the circulation system. Repair or replace the disinfection system so that the pool is continuously being disinfected. Ensure that the device has been approved by ODH for installation and operation.
3701-31-04 (E)(2) / The public swimming pool has safety equipment visible
Observed that the first aid kit for the pool was not accessible and/or visible.
A well stocked first aid kit shall be readily visible from the public pool and easily accessible. Place the safety equipment in a clearly visible area and ensure that the equipment is accessible in the event of an emergency.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
Observed that the ring bouy was not attached to the rope.
An approved type IV personal flotation device (ring bouy, throw bag, or equivalent device) with one quarter inch diameter line shall be no less than 30ft or more than 60ft in length. Provide a proper floatation device at all times. Attach ring bouies to the provide ropes.

Comments
According to OAC 3701-31-04 (4) (1), critical violations, the licensee shall not operate a public swimming pool when an imminent health risk is present including the lack of a disinfection system. Operator voluntarily shut down the pool. Operator was instructed to call us when the problems have been resolved.
