3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining operational records.observed operator is not keeping records of testing on sunday and mondays. observed past records lacking chlorine levels.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Observed that the pool is being tested at around 8AM on most days. The pool is open from 10AM to 10PM. The pool is not being tested within 12 hours after the first test.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use. Continue testing the before bathers use the pool but add another testing time throughout the day that is within 12 hours of the first test.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Observed standing water on the pool deck by the window to the workout area.
Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Observed that the emergency exit of the pool does not self latch when closing.
The pool shall have a complete perimeter barrier with self closing and latching doors or gates. Repair the door so that it self latches.
3701-31-04 (B)(6)(z) / Special features are maintained in good repair and according to manufacturer specifications.
Observed excessive rust and grime on handicap lift.
Special feature shall be maintained in good repair. Clean the lift more frequently and thoroughly.
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Emergency telephone was not working.
A working emergency phone shall be supplied to the pool. Repair the phone and place an out of order sign on the existing phone until it is repaired. The out of order sign must direct patrons to the nearest working phone that they can use to call for emergency help.

3701-31-04 (B)(1)(k)/Chemical storage or use shall not result in imminent health hazard.
The chemical storage and pump room do not lock to prevent access.
Chemicals shall be stored so they are unaccessible to unauthorized persons. The operator should not operate the pool during times when chemical storage is not maintained in a safe manner. Repair the door so that chemicals are unaccessible to unauthorized persons.
