[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Observed that the time of tested was not recorded on the logs.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. Record the time of testing so that it can be verified that the water is tested before bathers enter and thereafter.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Observed that the combined and total chlorine was not tested daily at spa prior to use.
Combined chlorine shall be tested daily prior to bathers entering the spa. Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. Record the combine and total chlorine levels on the daily logs.
3701-31-04 (B)(4)(a)(iv) / Water temperatures are recorded daily
Observed that the water temperatures were not being recorded daily.
Water temperature shall be recorded no less than once a day. Take and record the water temperature at least once/day.
3701-31-04 (B)(4)(a)(v) / Water clarity are not present
Observed that water clarity checks were not recorded on the daily logs.
Water clarity shall be assessed continuously and recorded on the daily logs. Do not operate the pool while the water is unclear. Determine cause of the lack of clarity and immediately correct the issue prior to bathers using the public swimming pool.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Observed that there was no backflow prevention device on the hose bibb in the pool pump room.
Cross connection shall be prevented. Remove all cross connection hazards from the pool area. Install the proper backflow prevention device.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Observed that the turnover rate was not sufficient.
The circulation system turnover rate shall be maintained continously twenty four hours a day. Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (D)(7) / Public spa shall have a functional automatic chemical controller
Critical Observed that the spa did not have an automatic chemical controller.
The licensee of a public spa shall provide an ACC to continually monitor and adjust level of free chlorine and the pH value of the public spa. Repair or replace the ACC for the public spa. If replacing be sure to use a system that has been approved by the ODH.
3701-31-04 (E)(2) / The public swimming pool has safety equipment visible
Observed that the first aid kit for the pool/spa was not accessible and/or visible.
The safety equipment shall be readily visible from the pool and easily accessible. Place the safety equipment in a clearly visible area and ensure that the equipment is accessible in the event of an emergency.
3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
Observed that the ring buoy was not attached to the rope.
An approved type IV personal floation device (ring buoy, throw bag or equivalent throwing device) with a one-quarter inch diameter line that in no less than 30ft and no more than 60ft in length. Provide a proper floatation device at all times. Attach the rope to the buoies.

According to OAC 3701-31-04 (B) (1) (d) (e), critical operational items, the licensee shall not operate a public swimming pool /spa when an imminent health hazard is present, including the lack of disinfection system and a lack of an automatic chemical controller. Operator voluntarily closed down the pool/spa. Operator was instructed to call us when the problems have been resolved.
