[?] A summary of the violations found during the inspection are listed below.

Call the Ohio Department of Health (ODH) at 614-466-3543 to determine if you must submit an Equipment Replacement Notification Report. If required, complete the form and submit it to the ODH at the address listed on the bottom of the form. Provide a copy to the health district at 937-390-5625. Please call the health district at 937-390-5600 ext 245 if you have any questions.
3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Equipment was not maintained per manufacturers specifications. A noticeable leak was observed, either in the white PVC piping near the pump or in the connection of the clear piping to the chlorine supply.
All equipment must be maintained in accordance with the manufacturers specifications. Replace any damaged PVC piping or inadequate connections.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
Depth markers/deck warning signs were not maintained. Several markers are loose and no longer well-sealed to the pool deck.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8. The pH was measured at 7.1.
Treat the pool water so that the pH is within the acceptable range. Adjust the pH so that it is between 7.2 to 7.8. It was indicated during the inspection that pH down was added to the pool because the pH was too high without calculating the volume of pH down to add. Always follow the directions on the chemical bottles to calculate the proper volume before adding chemicals to the pool to avoid overtreating. Arrangements to obtain the chemical needed to raise the pH were made during the inspection.
3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm. The alkalinity was measured at 55 ppm.
Raise the total alkalinity to at least 60 ppm. Arrangements to obtain the chemical needed to raise the alkalinity were made during the inspection.
