3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Observed suction outlet cover installed on end of PVC pipe that comes through pump room wall and turn 90 degrees extending into spa.
To reduce potential health hazard- a covering for any main drain or other suction outlet that is not properly installed,securely fastened or in compliance with the requirements with ANSI/ASME a112.19.8-2007; Due to lack of installation plan submital prior to installation it currently can not be determined if VGB cover was installed properly. Owner must receive approval from Ohio Department of Health for the installation of the drain cover.
3701-31-04 (B)(6)(l) / Secondary means of entrapment/evisceration prevention is provided
Observed no secondary means of entrapment/evisceration prevention.
In order to to reduce entrapment/evisceration - Submit plans to the Ohio Department of Health for review and approval for the secondary means that will be employed on the pool. To install one of either 1.second main drain. 2.gravity flow drainage system with unblockable outlet cover. 3.unblockable outlet cover with a compatible sump;or a SVRS that is properly installed.

No records of plans being submitted for substantially altering spa are at health department,Ohio department of health,or provided by owner. 3701-31-05 (a) no person shall construct,install,or otherwise substantially alter a public swimming pool-spa until the plans have been submitted to and approved by the director of health. Submit plans to Ohio Department of Health for the alteration of the spa.
