Repeat Observed the licensee /authorized representative was not keeping or maintaining operational records for the automatic chemical controller.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Repeat Observed that the handicap assistance chair was not maintained in good repair or in accordance with the manufacturer specifications.
Repair/maintain all the special features in good repair and in accordance with the manufacturer's specifications. If necessary, replace broken components.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Observed the exit door leading to the outside of the facility was not self-closing.
In order to reduce the risk of unauthorized access and risk of physical harm, ensure that the door is repaired in a way that it is self-closing.

3701-31-04 (B)(1)(e)/Automatic Chemical Controller (ACC) shall function properly.
Observed automatic Chemical Controller (ACC) exhibited an alarm at the time of inspection.
Do not operate the pool when the ACC unit is not functioning properly. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation.
3701-31-04 (B)(6)(t)/Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Observed that the chemical storage area door is not maintained locked.
In order to reduce the risk of unauthorized access, ensure that the door to the chemical room is not accessible to unauthorized personnel.
3701-31-04 (C)(3)/Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm.
Raise the total alkalinity to at least 60 ppm.
3701-31-04 (E)(2)(e)/Working emergency telephone/call box provided.
Observed emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
3701-31-04(D)(6)(a)(2)/Pool free chlorine disinfectant residual shall be above 1 ppm.
Observed pool free chlorine disinfectant residual was below 1 ppm.
Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.
