3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. Observed one ladder to be much more stable (with a replacement tread) and the other ladders to have replacement treads but to be still very shaky.
In order to reduce the risk of accidents, it is necessary that the ladders be maintained in good condition. Repair or replace the ladders/stairs/steps/connections so that they are slip resistant and stable.

The following violation(s) have been corrected since the last inspection.
3701-31-04 (B)(6)(i)/A means of determining rate-of-flow on all water pumps
A means of determining rate-of-flow was not found for all water pumps. Observed that the flow meter did not work properly (it appeared to be stuck by hard water deposits) so it was not possible to determine the rate of flow.
In order to ensure proper flow in the pool, it is necessary that a working flow meter be provided. Properly install a flow meter or have a pump curve available for every water pump used for the pool.
3701-31-04 (E)(2)(a)(ii)/A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 - 60 foot throw line was not provided at the unguarded pool. Observed the existing flotation device to be torn and not usable AND to not be attached to a rope of the correct length.
In order to ensure that emergency actions can be taken to avoid drowning, it is necessary that a flotation device in good condition AND properly attached to a rope of the proper length be provided at all times. Replace the damaged flotation device with a new one and ensure that it is securely attached to a rope of 30 - 60 feet in length.
3701-31-04 (E)(2)(d)/First aid kit provided
First aid equipment was not provided. Observed the first aid kit to be empty.
In order to ensure that patrons can adequately manage injuries, it is necessary that an adequately stocked first aid kit be provided. Provide first aid equipment, including a first aid kit, at the pool at all times.
3701-31-04 (E)(2)(e)/Working emergency telephone/call box provided.
Emergency device (telephone) was not working. Observed a non-functioning emergency telephone in the pool area. When the telephone was used, it did not connect to the front desk or an outside line. When the PIC was able to reach the front desk, the voice on the other end of the telephone was not able to be heard.
In order to ensure rapid access to emergency services, it is necessary that an adequately functioning telephone be provided. Discussed this with the PIC, who indicated that the telephone would be repaired or replaced. Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
3701-31-04 (D)(7)(c)/Audible/visual alarm shall activate when unit is malfunctioning or when pH or ORP levels are not acceptable.
The ACC unit was not activating an audible or visual alarm when unit malfunctioned or when pH or ORP levels were not acceptable. The ACC did appear to have a visual alarm, but it was difficult to see it in the pump room. The Ecolab person (Eugene) indicated that the unit was supposed to send a fax to the front desk. The front desk did not appear to have received a fax.
In order to ensure that the operator knows when the ACC is not working, it is necessary that the unit have an alarm. If the visual alarm is not sufficient to notify the operator (and if a fax is not sent to the front desk to inform the operator), it is necessary that an audible alarm be provided. Have the ACC unit serviced or replaced so that the audible or visual alarm is activated when necessary.

It was indicated during the inspection that the ladders would be further repaired and that a new telephone had been ordered and would be installed to provide a louder and more easily heard connection with the front desk.
