3701-31-04 (B)(1)(b) / SVRS device(s) shall be functional.
Critical Repeat Observed that the SVRS device that is being used as a secondary means to prevent entrapment/evisceration was not functional at the time of the inspection.
In order to reduce the risk of entrapment, ensure that the SVRS is functioning. Repair/replace the SVRS.
3701-31-04 (B)(6)(j) / All SVRS devices are maintained/ functional and tested
Critical Repeat Observed that the SVRS device was not maintained or not functional.
In order to reduce the risk of entrapment, ensure that the SVRS is maintained and functioning. Restore all SVRS devices to proper working order to prevent entrapment or evisceration.
3701-31-04 (D)(8)(b) / FAS-DPD test kit used (when standard DPD test kit insufficient)
Observed that the test kit on site was not able to test the concentrations of the chlorine.
In order to reduce the risk of potential health hazards, enure that a FAS-DPD test kit is used. Obtain a FAS-DPD test kit immediately.

ODH stated that the SVRS was installed in 2012 and therefore is there for a reason. Please repair, replace or install the StingL SVRS. Will follow up in a week.
