3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED NON-WORKING LIGHTING AT REACH-IN COOLER.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.7(C) / Storage of soiled linens - methods.
Soiled linens were not kept in approved location. OBSERVED SOILED WIPING RAG BEING STORED IN RESTOOM.
Soiled linens shall be kept in clean, nonabsorbent receptacles or clean, washable laundry bags and stored and transported to prevent contamination of food, clean equipment, clean utensils, and single-service articles or single-use articles.

Wiping cloths are being air-dried in an unapproved location. OBSERVED SOILED WIPING RAG BEING AIR-DRIED IN RESTROOM.
Wiping cloths laundered in an FSO or RFE that does not have a mechanical clothes dryer shall be air-dried in a location and in a manner that prevents contamination.

Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED CRACKED AND MISSING TILES IN ON FACILITY FLOOR.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. OBSERVED TOILET BASE IN BOTH EMPLOYEE RESTROOMS THAT WERE NOT CAULKED.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. TO PREVENT MOLD GROWTH FROM MOISTURE BUILD-UP THE PIC SHALL CAULK/SEAL TOILET BASES.

The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
