Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED SOILED, MISSING AND BROKEN CEILING TILE THROUGHOUT THE FACILITY.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE ALL SOILED, MISSING AND BROKEN CEILING TILE THROUGHOUT THE FACILITY.

Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT INSIDE OF THE MEN'S AND WOMEN'S RESTROOMS.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. THOROUGHLY CLEAN THE INSIDE OF THE MEN'S AND WOMEN'S RESTROOMS TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED EMPTY BOXES AND UNUSED EQUIPMENT IN THE FACILITY.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE ALL EMPTY BOXES AND ALL UNUSED EQUIPMENT THROUGHOUT THE FACILITY TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE.
