Repeat Physical facilities not maintained in good repair. Observed one partially detached door entrance gasket to the walk-in freezer. Repair to properly restore the observed partially detached door entrance gasket which is located to the walk-in freezer door.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed one missing ceiling tile located by the walk-in freezer and one smaller wall area missing material near the rear entrance door location. resurface to properly restore the observed missing ceiling tile which is ocated by the walk-in freezer and one smaller wall area missing material near the rear entrance door location.
