OBSERVED IMPROPERLY PLACED TIME-TEMPERATURE CONTROLLED FOR SAFETY (TCS) FOODS: OBSERVED RAW, FROZEN CHICKEN PLACED ABOVE READY TO EAT FOODS IN THE REACH IN FREEZER.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. THE CHICKEN WAS REMOVED AND PLACED IN A DIFFERENT LOCATION AT THE TIME OF INSPECTION. Correct By: 13-Nov-2015

Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. OBSERVED TCS FOODS STORED IN THE REACH IN COOLER THAT WERE NOT DATE MARKED.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. THE FOODS WERE DATE MARKED AT THE TIME OF INSPECTION.

Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED DETERIORATED CEILING TILE ABOVE THE DISPLAY WARMER AND MISSING CEILING TILES IN THE RETAIL WAITING AREA.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

THIS FACILITY IS IN VIOLATION OF CODE 3717.41 LICENSE REQUIRED FOR FOOD SERVICE OPERATION - SEPARATE LICENSES. NO PERSON OR GOVERNMENT ENTITY SHALL OPERATE A FOOD SERVICE OPERATION WITHOUT A LICENSE. A SEPARATE LICENSE IS REQUIRED FOR EACH FOOD SERVICE OPERATION A PERSON OR GOVERNMENT ENTITY OPERATES. AS A RESULT, THIS OPERATION MUST CEASE AND DESIST ALL ACTIVITY UNTIL A FOOD SERVICE LICENSE IS OBTAINED. THE ABOVE NOTED VIOLATION MUST BE COMPLETED BY NOVEMBER 24, 2015 TO AVOID PROSECUTION.
