3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. OBSERVED A BUILD-UP OF FOOD PARTICLES BESIDE AND BELOW THE GRILL
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 21-Jul-2015

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Food storage containers are not properly labeled. OBSERVED AN UNMARKED CONTAINER CONTAINING A WHITE & ORANGE SUBSTANCE.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized. MARK THE CONTAINER TO IDENTIFY THE ITEM WITHIN THE CONTAINER.
3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper use and/or maintenance of wiping cloths.
(1) Cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose; (2) Cloths that are in use for wiping counters and other equipment surfaces shall be: (a) held between uses in a chemical sanitizer of the appropriate concentration; and (b) shall be laundered daily; (3) Cloths that are used with raw animal foods shall be kept separate from cloths used for other purposes; (4) Dry wiping cloths and the chemical sanitizing solution in which wet wiping cloths are held between uses shall be free of food debris and visible soil; (5) Containers of chemical sanitizing solutions specified in paragraph (M)(2)(a) of this rule in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service articles, or single-use articles; (6) Single-use disposable sanitizer wipes shall be used in accordance with EPA-approved manufacturer's label use instructions.
3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. OBSERVED THAT A NON-COMMERCIAL GRADE COOLING UNIT IS IN USE.
Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.
3717-1-04.2(D) / Ventilation hood systems - adequacy.
Observed an accumulation of grease or condensation from ventilation hood system.
Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
There is no test kit available for measuring the concentration of the sanitizer. OBSERVED AN EXPIRED TEST KIT (EXPIRED ON 06-30-2015) ON PREMISE.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment did not appear to be properly sealed or spaced for cleaning. OBSERVED THAT THE THREE COMPARTMENT SINK WAS NOT CAULKED TO THE WALL.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule. CAULK THE THREE COMPARTMENT SINK TO PREVENT FOOD PARTICLES/DIRT FROM GETTING TRAPPED BEHIND THE WALL.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED BROKEN AND MISSING FLOOR TILE IN THE FACILITY AROUND THE RETAIL STORE UNIT AND IN THE RESTAURANT (KITCHEN AREA).
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

THIS FACILITY IS IN VIOLATION OF CODE 3717.41; LICENSE REQUIRED FOR FOOD SERVICE OPERATION - SEPARATE LICENSES. NO PERSON OR GOVERNMENT ENTITY SHALL OPERATE A FOOD SERVICE OPERATION WITHOUT A LICENSE. A SEPARATE LICENSE IS REQUIRED FOR EACH FOOD SERVICE OPERATION A PERSON OR GOVERNMENT ENTITY OPERATES.
