Repeat Facility does not have a person with level two certification in food protection.
AT LEAST ONE PERSON IN MANAGEMENT CAPACITY WHO HAS AUTHORITY IN DIRECTING FOOD SERVICE MUST TAKE A FIFTEEN HOUR OHIO DEPARTMENT OF HEALTH (ODH) APPROVED FOOD SAFETY TRAINING COURSE. UPON COMPLETION, HAVE ALL REQUIRED DOCUMENTATION SENT TO ODH FOR REVIEW AND POSSIBLE APPROVAL TO RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.

Plumbing system not properly maintained or repaired. OBSERVED LEAK FROM UNDERNEATH THE THREE COMPARTMENT SINK AND THE ADDITIONAL COMPARTMENT FOR HANDWASHING.
MAKE NECESSARY REPAIRS TO ABATE THE LEAK FROM UNDERNEATH THE SINK. MAINTAIN AT ALL TIMES.

MAKE NECESSARY ADJUSTMENTS TO THE WOOD BACKSPLASH IN ORDER TO PROVIDE A SMOOTH, NON-ABSORBENT, EASILY CLEANABLE SURFACE. MAINTAIN AT ALL TIMES.
