Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION WAS NOT AVAILABLE.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN APPROVED FOOD SAFETY TRAINING COURSE, OBTAIN THE CERTIFICATE UPON COURSE COMPLETION, THEN CONTACT THE OHIO DEPARTMENT OF HEALTH FOR ADDITIONAL INFORMATION TO RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. OBSERVED DIRT DEBRIS BUILD-UP ON THE FAN GUARD INSIDE OF THE WALK IN COOLER.
Nonfood-contact surfaces of equipment shall be kept clean. THOROUGHLY CLEAN THE ENTIRE WALK IN COOLER (FLOORS, WALLS, CEILINGS AND FAN GUARDS).

Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED MISSING AND LOOSE FLOOR BASE COVE UNDERNEATH THE DISHWASHER.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE THE MISSING FLOOR BASE COVE.
