Improper use and/or maintenance of wiping cloths. OBSERVED SOILED WIPING CLOTHS IN THE EMPTY 3 COMPARTMENT SINK.

Warewashing equipment and/or components not cleaned at required frequency. OBSERVED THE 3 COMPARTMENT SINK HAD COMPARTMENTS THAT WERE SOILED.

Non-food contact surfaces of equipment are unclean. OBSERVED THE INSIDE OF THE REACH IN BEER COOLER WAS SOILED WITH MOLD. DISCARD ALL BEER BOTTLES IN THE REACH IN COOLER AND THOROUGHLY WASH AND SANITIZE THE INSIDE OF THE COOLER.

Physical facilities not maintained in good repair. OBSERVED DAMAGED CEILING TILES IN VARIOUS LOCATIONS THROUGHOUT THE FACILITY.

Unnecessary or nonfunctional items and /or litter on premises. OBSERVED UNNCECESSARY ITEMS SUCH AS A WASHER, DRYWALL, BUILDING MATERIALS, AND OLD NON-WORKING OR BROKEN COOLERS.

Facility not maintained clean. OBSERVED THE FACILITY IS SOILED AND NEEDS TO BE DUSTED AND CLEANED. THE PIC STATED THE FACILITY HAS NOT BEEN OPEN FOR A FEW WEEKS.
