Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events..
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.( VOMIT AND FECAL GUIDANCE HAND OUT GIVEN AT TIME OF INSPECTION)
3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items.( OBSERVED SNACK CHIPS OFFER FOR RETAIL SALE STORE ON THE FLOOR IN THE RETAIL AREA OF THE OPERATION)
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Observed a handwashing sink without water at the required temperature.( OBSERVED HAND SINK LOCATED IN THE EMPLOYEES RESTROOM NOT AT THE REQUIRE TEMP.)
To ensure proper cleaning of hands, a handwashing sink shall be equipped to provide water at a temperature of at least 100° F (thirty-eight degrees Celsius) through a mixing valve or combination faucet.( PROVIDE THE NECESSARY ADJUSTMENT AND/OR REPAIRS MAINTAIN AT ALL TIMES)

Observed an outer opening, window, or door without proper protection against the entry of insects and rodents.(OBSERVED REAR STORAGE DOOR WITH GAP AT BOTTOM)
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.( PROVIDE THE NECESSARY REPAIRS IN REGARDS TO DOOR MAINTAIN AT ALL TIMES)

The physical facilities are not being maintained in good repair.( OBSERVED WATER SOILED AND MOLDY CEILING TIMES IN THE OPERATION)

Repeat Observed a build-up of dirt and debris.( OBSERVED BUILD-UP OF DIRT ON FLOORS IN THE RESTROOM AND BACK STORAGE AREA AT TIME OF INSPECTION)
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises.( OBSERVED NONFUNCTIONAL ITEMS STORED IN THE SERVICE SINK AND LITTER ON PREMISE AT TIME OF INSPECTION)
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.( REMOVE ITEMS WHERE IS NEEDED MAINTAIN AT ALL TIMES)
