Improper storage of food items. OBSERVED RAMEN NOODLES AND CHIPS IN BOXES ON THE GROUND. PIC WAS INSTRUCTED TO PLACE PALLETS UNDERNEATH THEM TO ELVELATE THEM AT LEAST 6 INCHES FROM THE GROUND. OBSERVED THE CHIP RACK BOTTOM RACK IS LESS THAN 6 INCHES FROM THE GROUND. PLEASE ELEVATE CHIP RACK.

Equipment components are not intact, tight or properly adjusted. OBSERVED THE WALK IN COOLER GASKET WAS PEELING AWAY FROM THE DOOR AT THE BOTTOM. PLEASE REPAIR GASKET.

Non-food contact surfaces of equipment are unclean. OBSERVED THE ICE CREAM FREEZER CLOSEST TO THE DOOR IS IN NEED OF CLEANING. PLEASE WIPE OUT THE INSIDE OF THE ICE CREAM FREEZER TO RID IT OF DIRT AND DEBRIS.

Physical facilities not maintained in good repair. OBSERVED SEVERAL CEILING TILES IN THE FACILITY THAT ARE WATER DAMAGED DUE TO A LEAKING ROOF. OBSERVED ALMOST ALL THE CEILING TILES ARE MISSING FROM THE BATHROOM CEILING. PLEASE REPLACE ALL DAMAGED AND/OR MISSING CEILING TILES.
