Critical PIC unable to demonstrate knowledge by having no critical violations. OBSERVED A CRITICAL VIOLATION IN THE FACILITY AT THE TIME OF THE INSPECTION. IN ORDER TO PREVENT CRITICAL VIOLATIONS, THE PIC SHALL MAINTAIN MANAGERIAL CONTROL OVER THE FACILITY AT ALL TIMES.

Critical Presence of live insects, rodents, and other pests. OBSERVED A LIVE ROACH IN THE FACILITY BY THE 3 COMPARTMENT SINK. IN ORDER TO PREVENT PESTS, PROFESSIONAL PEST CONTROL SERVICES SHOULD BE INCREASED UNTIL THE ISSUES ARE RESOLVED. PLEASE HAVE PEST CONTROL FOR AN ADDITIONAL VISIT BY MONDAY FEB. 14TH.

Corrected During Inspection No towels or drying device at the handwashing sink(s). OBSERVED NO PAPER TOWELS IN THE BATHROOM AT THE HANDWASHING SINK.
