Comments
There was a fire reported to Cleveland Department of Public Health by the Division of Fire on 5/7/18. The facility closed for the evening. The fire was located on the back regulator of the stove in the kitchen food prep area. The ansul system did not activate. The fire was reported by the PIC as small and contained to the back of stove. The gas was shut off to extinguish. There was food exposed near the fire that was voluntarily discarded. (see voluntary destruction form attached) The discarded food was from the cooler under the stove and the grill adjacent to the stove. The equipment and surrounding area was cleaned and sanitized at time of inspection.

FSO approved to re-open.
