Critical There is no test kit available for measuring the concentration of the sanitizer. OBSERVED THAT THERE WAS NO SANITIZER NOR A TEST KIT AVAILABLE.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided. PROVIDE BOTH SANITIZER AND A TEST KIT TO SANITIZE LUNCH TRAYS AND ANY UTENSILS, ETC. Correct By: 23-Jun-2015

Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED BROKEN TILES ON THE KITCHEN FLOOR.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPAIR ANY/ALL TILES SO THAT THE FLOORS CAN BE EASILY CLEANED.
