THE PERSON IN CHARGE CURRENTLY HAS A FOOD MANAGER CERTIFICATE (# 11699445; EXPIRES ON 11-12-2019). HAVE ALL REQUIRED DOCUMENTATION SENT TO OHIO DEPARTMENT OF HEALTH (ODH) FOR REVIEW AND POSSIBLE APPROVAL TO RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.

Non-durable equipment observed. OBSERVE THAT THE RIGHT SIDE TWO-DOOR REACH IN COOLER HAS AN AMBIENT TEMPERATURE OF 45 DEGREES FAHRENHEIT.
MAKE NECESSARY ADJUSTMENTS TO THE REACH IN COOLER FOR IT TO OPERATE AT AN AMBIENT TEMPERATURE NO HIGHER THAN 41 DEGREES FAHRENHEIT AT ALL TIMES.
