FLY POPULATION HAS DECREASED SINCE INITIAL INSPECTION BUT IS STILL AN ISSUE. PEST CONTROL COMPANY HAS MADE RECOMMENDATION FOR ELECTRONIC FLY CONTROL DEVICES.

Insufficient air gap between the flood rim and the water supply inlet. OBSERVED DRAINLINES FOR PREP SINK AND PEPSI DISPENSER TO TERMINATE INTO INLET PIPES. TO REDUCE HAZARD OF BACKFLOW DRAINLINES MUST TERMINATE ABOVE SEWAGE INLETS AT A DISTANCE OF AT LEAST 2 TIMES THE DIAMETER OF THE OUTLET PIPE.

PIC unable to demonstrate knowledge by having no critical violations. OBSERVED CRITICAL VIOLATIONS AT THE TIME OF INSPECTION. PERSON IN CHARGE MUST MAINTAIN OVERSIGHT OF OPERATION TO ENSURE THAT CRITICAL VIOLATIONS DO NOT OCCUR.
