3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED THAT THE THREE DOOR REACH IN COOLER IS NOT WORKING. THE MANAGER EXPLAINED THAT THE UNIT STOPPED WORKING TWO DAYS AGO. THERE WAS NO TIME AND TEMPERATURE CONTROLLED FOR SAFETY (TCS) FOODS STORED INSIDE OF THE UNIT, BUT OBSERVED MILK, EGGS, CREAM CHEESE, PRE-PACKAGED SLICED MEATS STORED INSIDE OF THE REACH IN FREEZER NEXT TO THE REACH IN COOLER. THE MANAGER EXPLAINED THAT THE MENTIONED FOOD ITEMS ARE NOT FOR SALE AND IS WAITING FOR THE SUPPLIER TO REMOVE THEM.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. DISCONTINUE SELLING THE ABOVE MENTIONED ITEMS STORED INSIDE OF THE REACH IN FREEZER. HAVE THE REACH IN COOLER REPAIRED/SERVICED IN ORDER FOR IT TO OPERATE AT AN AMBIENT TEMPERATURE NO HIGHER THAN 41 DEGREES FAHRENHEIT AT ALL TIMES. CONTACT THE CLEVELAND DEPARTMENT OF PUBLIC HEALTH FOR THE SANITARIAN TO VERIFY THE REACH IN COOLER'S AMBIENT TEMPERATURE BEFORE USE.

OBSERVED POTHOLES ON THE FACILITY'S PARKING LOT.
HAVE THE PARKING LOT SERVICED IN ORDER TO AVOID POSSIBLE STANDING WATER.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED SOILED AND MISSING CEILING TILES THROUGHOUT THE FACILITY.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE ALL SOILED AND MISSING CEILING TILES THROUGHOUT THE FACILITY.
