Observed a FSO that did not display their license at the facility; Observed a mobile FSO that did not display all required information on the exterior of the unit. OBSERVED THAT THERE WAS NO FOOD SERVICE OPERATION LICENSE POSTED AT THE FACILITY.
A licensee shall display the license for that food service operation at all times at the licensed location; Each operator of a mobile FSO shall conspicuously display the name of the operation, city of origin, and area code and telephone number on the exterior of the mobile unit with the individual lettering measuring at least three inches high by one inch wide. PROVIDE AND POST THE CURRENT FOOD SERVICE OPERATION LICENSE IMMEDIATELY. Correct By: 01-May-2018

Observed a handwashing sink without water at the required temperature. OBSERVED THAT THE UNISEX RESTROOM HANDSINK'S WATER TEMPERATURE WAS AT 45 DEGREES FAHRENHEIT.
To ensure proper cleaning of hands, a handwashing sink shall be equipped to provide water at a temperature of at least 100° F (thirty-eight degrees Celsius) through a mixing valve or combination faucet. MAKE NECESSARY ADJUSTMENTS/REPAIRS TO THE HANDSINK IN ORDER FOR IT TO PROVIDE RUNNING WATER AT OR ABOVE 100 DEGREES FAHRENHEIT AT ALL TIMES. Correct By: 04-May-2018

Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED A NON-WORKING TWO DOOR REACH IN COOLER IN THE FACILITY.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE THE NON-WORKING TWO DOOR REACH IN COOLER TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE. Correct By: 15-May-2018
