| Upon a thorough search throughout the FSO observed two live roach insect. Note: an extension is based on sanitarian discussion with management confirming materials and full elimination of harborage crevices which is located near the rear food preparation area. Note: Observed and confirmed with management of Ecolab yesterday's pest application which also included a powder application located near this area in the process for full elimination. In addition, Ecolab which provides the weekly exterminating applications is available at contact phone number: 1-800-325-1671. |
