3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical The handwashing sink was not easily accessible. Handsink completely blocked by miscellaneous equipment in rear kitchen.
To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use. Handsinks must remain accessible, stocked and in good working order at all times.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.Class III operation requires at least one trained individual at this level.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.Operation no longer preparing foods. Downgrade at next renewal, or provide a trained individual that meets this requirement.
3717-1-03.2(I)(1) / Storage or display of food in contact with water or ice - packaged foods
Observed packaged foods stored or displayed directly in contact with ice and/or water. Walk-in cooler has ceiling leak, with water dripping onto containers and shelving.
Packaged food may not be stored in direct contact with ice or water if the food is subject to the entry of water because of the nature of its packaging, wrapping, or container or its positioning in the ice or water. Prevent leaking water from condenser or ceiling from contacting food containers.
3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items.Food and beverages stored on floor in walk in cooler and other areas.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Properly sroe all containers of food and beverages as required by code.
3717-1-03.2(Y) / Miscellaneous sources of contamination.
Miscellaneous sources of contamination observed. Significant leaks in many areas from roof.
Food shall be protected from contamination that is not specified under 3717-1-03.2 (A) to (X). Repair immediately to prevent leaks into all areas of operation to protect packaed items from contamination from premises. Remove all unapproved materials (chemicals, gasoline, and other items) from operation to protect from accidental contamination of containers of food and beverages.
3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces are not designed and constructed to allow easy cleaning or maintenance. Wood used inside cooler to repair surface and for shelving.
Nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance. Provide smooth, durable easily cleanable surfaces in all areas, and repair surfaces only with approved materials. Maintain in good condition at all times.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. Ice machine, deli cooler in non-working condition. Freezer not functioning (repaired recently per manager, but not operating at time of inspection. Interior temperature 82F).
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair all equipment in operation to restore to proper working condition, or remove from operation.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are dirty. Shelving and racks in walk-in cooler dirty with build-up mold and dirt.
Nonfood-contact surfaces of equipment shall be kept clean. Thoroughly clean all coolers and maintain clean routinely to prevent buildup of mold, dirt and debris.
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors, walls, and/or ceilings that were not constructed of approved materials.Raw wood and other materials used to repair areas, and many ceiling, wall and floor areas with significant damage.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture.Provide a smooth durable easy to clean surface constructed of approved materials in all areas of operation.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. Missing and crumbling tiles in walk in cooler and in many areas of operation.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Floors, walls and ceiling in all areas need to be maintained in sound condition to allow for routine cleaning and maintenance of all surfaces, and cleaned routinely.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. Flooring in many areas with no coving, standing water from leaking roof.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. Repair floors to ensure they are in good condition and properly sealed and coved as required by code.
3717-1-06.1(S) / Private homes and living or sleeping quarters - use prohibition.
A private home or living or sleeping quarters was being used for operating a FSO or RFE. Rear exit door area had couch/daybed.
To prevent foodborne illness, except as provided by rule 3717-1-20 of the Administrative Code, a private home, a room used as living or sleeping quarters, or an area directly opening into a room used as living or sleeping quarters may not be used for conducting FSO or RFE operations. Remove unapproved couch/daybed from rear area.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. Serious building conditions showing disrepair in many areas.
The physical facilities shall be maintained in good repair. Repair all areas to return operation to good condition (roof leaks and damaged floors, walls and ceilings throughout operation).
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary or nonfunctional items and /or litter on the premises. Lawnmower, chainsaw, and other miscellaneous equipment, junk and debris stored in operation, preventing access to handsink, 3 compartmnt sink, and preventing routine cleaning and maintenance.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. Remove all junk, debris, and materials not related to the retail food operation. Maintain premises free of litter, debris, and unnecessary items.

DOWNGRADE TO CLASS II at renewal.
Operation no longer performing level III activities per operator and manager.
Operation in serious disrepair (roof in rear leaking into buckets and trashcan and into inside of walk-in cooler). Gas-powered equipment stored in kitchen (lawnmower, chainsaw). Operator (Samia Hildi) contacted by phone, agreed to voluntarily close to repair and address all violations. Spoke with Manager via phone, also advised of conditions and agreed to volunrarily cease operating to correct all violations from prior inspection and the violations noted on today's inspection.
