Observed insects and/or rodents in the facility's waste receptacles for refuse or recyclables. OBSERVED FRUITFLIES FLYING AROUND THE TRASH CAN CONTAINING EMPTY BEER CANS.
Refuse, recyclables, and returnables shall be stored in insect and rodent proof receptacles or waste handling units.

Observed an improperly cleaned storage area for refuse, recyclables, or returnables. OBSERVED DEBRIS (INCLUDING A TIRE) ON THE GROUND NEAR THE DUMPSTER.
A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. NOTICED BROKEN TILES ON FACILITY'S FLOOR IN THE MAIN AREA AND BEHIND THE BAR.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE/REPAIR ANY BROKEN TILE SO THAT THE FLOOR CAN BECOME EASILY CLEANABLE.
3717-1-06.1(L) / Toilet rooms - enclosed.
A toilet room was not completely enclosed and/or did not have a tight-fitting, self-closing door. BOTH THE MEN'S AND WOMEN'S RESTROOMS ARE MISSING THE ENCLOSING AROUND THE TOILET UNIT.
A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door,except where located outside an FSO or RFE, or does not open directly into the food preparation area.
